Registered Nurse Navigator - ENT

Role Overview: This position involves coordinating patient care, providing education, and ensuring continuity of care within a multidisciplinary healthcare environment. The role includes facilitating patient entry into care programs, supporting pre- and post-hospitalization processes, and acting as a liaison between patients and healthcare teams.

Key Responsibilities

• Coordinate intake and ongoing care for new and existing patients.
• Provide patient education to promote understanding of conditions and treatments.
• Support clinical activities, assist providers, and facilitate multidisciplinary clinic coordination.
• Guide patients through care programs, ensuring timely follow-up and surveillance.
• Communicate effectively with patients via various methods and coordinate support services, such as social and support groups.
• Educate team members and act as a resource within the care team.

Qualifications & Skills: Requires graduation from a specialty training program, with a preference for a Bachelor's Degree in Nursing. Strong communication, organizational, and patient advocacy skills are essential.
