Job Description
Role:
The Pharmacist provides safe, effective, and cost-efficient medication therapy, developing and monitoring evidence-based care plans focused on patient outcomes and quality of life.
Responsibilities
• Deliver pharmaceutical care within assigned patient care areas.
• Interpret and clarify physicians' medication orders.
• Review patient medication profiles for safety, effectiveness, and interactions.
• Recommend formulary alternatives and advise medical staff on drug therapy issues.
• Provide drug information to healthcare professionals and patient discharge consultations.
• Participate in rounds, calculate dosages, verify orders, and prepare sterile medications.
• Coordinate resource use, supervise pharmacy staff, and ensure compliance with standards.
• Assist with policy review, provide training, and resolve pharmacy-related issues with other healthcare teams.
Qualifications & Skills:
Bachelor's degree in Pharmacy required; Doctoral degree preferred. Strong knowledge of pharmacotherapy, excellent communication, attention to detail, leadership abilities, and proficiency in medication management are essential.
