Job Description
Role Overview: The Pharmacist provides safe, effective, and cost-efficient medication therapy, developing evidence-based care plans focused on patient outcomes and quality of life. They assess medications for indication, safety, effectiveness, and adherence, while monitoring laboratory results and documenting outcomes.
Key Responsibilities
• Deliver pharmaceutical care tailored to their assigned patient care area.
• Coordinate resources to ensure timely, efficient, and appropriate care.
• Interpret, clarify, and correct physicians' medication orders.
• Review medication profiles for contraindications, interactions, and duplications.
• Evaluate all medication orders for prescribing issues and suggest formulary alternatives.
• Consult with medical staff on drug therapy problems and review patient lab results.
• Provide drug information to healthcare professionals and discharge medication counseling.
• Participate in rounds, calculate drug dosages, and verify orders for accuracy.
• Conduct in-service training and prepare sterile medications using aseptic techniques.
• Report supply needs, supervise pharmacy staff and interns, and ensure compliance with departmental standards.
• Collaborate with medical and nursing teams to resolve pharmacy-related issues and assist in policy revisions.
Qualifications & Skills: Bachelor's degree in Pharmacy required; Doctoral degree preferred. Strong knowledge of pharmacotherapy, excellent communication, and supervision skills, along with the ability to work collaboratively in a clinical environment.
