Contract Days Onsite Split between two outpatient clinic locations

Key Responsibilities

- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
- Conduct 60-minute evaluations and 30-minute follow-up sessions in an outpatient setting
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
- Supervise and mentor students as assigned, including development of learning objectives and completion of student evaluations
- Adhere to discipline-specific State Practice Act and all applicable regulatory requirements; maintain active professional licensure in good standing


Required Qualifications

- Active Physical Therapist license in the state of Colorado
- Graduate of an accredited Physical Therapy program
- Ability to work across two outpatient clinic locations
- Compliance with all state-mandated continuing education requirements


Preferred Experience / Skills

- Prior experience in an outpatient physical therapy setting
- Experience with student supervision and clinical education
- Familiarity with performance improvement methodologies and quality initiatives
