Registered Nurse (RN) - Home Health

We are seeking a compassionate Registered Nurse (RN) for a home health role focused on delivering patient-centered care in patients' homes and via telehealth.

Key Responsibilities Include

• Performing in-home assessments, clinical interventions, and care coordination under the guidance of licensed providers.
• Conducting comprehensive safety evaluations, managing medication administration, wound care, and diagnostic procedures.
• Maintaining accurate documentation, supporting care transitions, and participating in interdisciplinary team meetings.
• Providing education to patients and caregivers, fostering positive relationships with community providers, and ensuring compliance with regulations.

Qualifications Include

• Active RN licensure, with 3+ years of direct patient care experience, preferably in home care, hospice, palliative, or geriatrics.
• CPR certification and a valid driver’s license with auto insurance.
• Proficiency in telehealth platforms, Microsoft Office, Electronic Health Records, and excellent communication skills.
• Ability to work independently, handle administrative tasks, and travel locally for patient visits.

This role does not involve supervisory duties and requires physical ability to lift up to 50lbs, stand for extended periods, and perform patient transfers.
