Registered Nurse Navigator - Nephrology

Role Summary: Provides comprehensive patient care coordination, education, and clinical support within a multidisciplinary healthcare setting. Acts as a liaison between patients and caregivers, facilitating entrance into care programs, and ensuring continuity of care before and after hospitalization.

Key Responsibilities

• Facilitates patient entry into care programs and coordinates multidisciplinary clinics.
• Provides patient education to promote understanding of conditions and ongoing care.
• Supports diagnosis and treatment by collaborating with healthcare providers.
• Coordinates patient activities and follow-up to ensure timely care and surveillance.
• Acts as a resource for patients and team members, guiding and reinforcing education.
• Coordinates support services, including social support and support groups.
• Maintains systems for effective communication, follow-up, and post-care surveillance.
• Provides nursing care and assists physicians with procedures.

Qualifications & Skills

• Minimum of 2 years related healthcare experience.
• Graduate from Specialty Training Program; Bachelor's Degree in Nursing preferred.
• Strong communication, organizational, and collaborative skills.
• Ability to educate patients and coordinate multidisciplinary efforts effectively.
