Registered Nurse Coordinator - Primary Care

Overview:

This role involves serving as an advanced nurse within an interdisciplinary team to promote preventative care, conduct screenings, and coordinate disease management while supporting primary care providers, patients, and families. The nurse assesses healthcare, educational, and psychosocial needs, develops individualized care plans, and facilitates effective communication among healthcare members to eliminate barriers.

Key Responsibilities:

Implement clinical interventions based on evidence-based guidelines, coordinate laboratory and diagnostic tests, document activities in electronic health records, and manage patient data. The role includes patient education on disease management, participating in professional development, and supporting care transitions. Additionally, the nurse assists in designing, implementing, and evaluating care coordination programs, manages medication administration, IV monitoring, and specimen collection, and provides guidance on treatment options.

Qualifications & Skills:

Requires at least two years of RN experience, preferably in Med/Surg settings. A graduate from a specialty nursing program is required; a Bachelor's Degree in Nursing is preferred. Strong communication, collaboration, and clinical skills are essential.
