Job Description
Role Overview: The Community Hospice Admission Nurse serves as the initial contact for clients entering hospice care, ensuring a personalized and respectful experience that aligns with client goals.
Key Responsibilities
• Coordinate admission processes by collaborating with intake, reviewing referrals, and scheduling in-person admissions.
• Engage with clients and caregivers to discuss hospice plans and organize care initiation.
• Manage charting and care coordination before transitioning responsibilities to the RN case manager and interdisciplinary team.
Qualifications & Skills
• RN license recognized by the relevant state.
• At least four years of experience as a Community Hospice Nurse or five years in home health nursing; hospice and palliative care certification preferred.
• Strong clinical experience in relevant areas.
