Job Description
Role: Palliative Care Nurse providing direct patient and family care through consultations for individuals with serious illnesses, focusing on those with a prognosis of one year or less who are not eligible for hospice.
Responsibilities Include
Applying evidence-based palliative medicine practices ethically and effectively across physical, emotional, spiritual, psychosocial, ethical, and safety domains, tailored to patient age.
Utilizing clinical expertise to develop comprehensive care plans based on palliative care principles.
Gathering detailed information from medical records, family, and caregivers, conducting thorough assessments, including physical, emotional, and spiritual evaluations.
Completing assessments, interpreting findings, managing medications, diagnosing issues, initiating therapeutic plans, and evaluating outcomes.
Qualifications
RN or APRN licensure with prescriptive authority in the state of practice.
Minimum of 5+ years of clinical nursing experience.
Current BCLS certification, valid driver's license, and reliable transportation.
Preferred Skills And Certifications
Advanced practice palliative care certification (ACHPN).
Experience as APRN in Hospice, Palliative Care, or Oncology.
CPR certification within 90 days of hire is desirable.
Compensation and Benefits: Salary ranges from $109,500 to $164,000, including base pay and potential bonuses, based on experience and performance.
