Job Description
Role: Nurse Practitioner providing home-based primary and palliative care to medically complex, home-limited adults.
Responsibilities
• Conduct in-home and telehealth visits to assess and manage patient health.
• Develop and implement patient-centered care plans.
• Manage chronic conditions and coordinate care with specialists.
• Educate patients and caregivers to promote optimal health outcomes.
• Document all care activities accurately in the EMR.
• Collaborate within an interdisciplinary team to improve patient outcomes.
• Reduce avoidable hospitalizations through proactive management.
Qualifications & Skills
• Licensed Nurse Practitioner with relevant clinical experience.
• Strong assessment, diagnostic, and care planning skills.
• Excellent communication and patient education abilities.
• Ability to coordinate care across multiple providers.
• Commitment to compassionate, patient-centered care and teamwork.
