Registered Nurse (RN) - Home Health - Full-Time

We are seeking a compassionate Registered Nurse (RN) for a home health role focused on delivering patient-centered care through in-home visits, telehealth consultations, and administrative tasks.

• Responsibilities include: conducting assessments, reviewing medications, performing clinical interventions (e.g., wound care, medication administration), documenting patient encounters, providing education, supporting care transitions, and collaborating with interdisciplinary teams.
• Supporting urgent and episodic interventions while fostering positive relationships with patients, caregivers, and healthcare providers.
• Maintaining compliance with regulatory standards and ensuring accurate documentation.
• Participating in team meetings, ongoing training, and community engagement to enhance patient care.

Qualifications And Skills

• Active state licensure (compact preferred) and CPR certification.
• Minimum of 3 years in a direct patient care role, with experience in home care, hospice, palliative, or geriatrics preferred.
• Proficiency in telehealth platforms, electronic health records, and Microsoft Office suite.
• Strong communication, organizational, and decision-making skills.
• Ability to work independently, travel locally up to 100%, and lift patients or equipment as needed.
