Role: Nurse Navigator RN
Responsibilities
Guide patients through complex care pathways in a dynamic environment
Coordinate appointments, referrals, and multidisciplinary services for seamless patient transitions
Conduct clinical assessments and provide patient and family education
Identify and address barriers to care to ensure optimal outcomes
Document all interactions accurately in EMRs, maintaining safety and quality standards
Collaborate with EMS and hospital partners to deliver compassionate, life-impacting care
Enhance specialized clinical skills through ongoing collaboration and learning
Qualifications & Skills
Registered Nurse (RN) license
Strong clinical assessment and patient education skills
Excellent coordination and communication abilities
Ability to work effectively in a fast-paced, team-oriented setting
Commitment to safety, quality, and compassionate care
