Job Description
SouthernCare New Beacon Hospice is on the lookout for a compassionate Registered Nurse Clinical Liaison to serve as a vital link between patients, families, and referral sources, ensuring access to exceptional hospice care. This field-based position will involve assessing patients for hospice eligibility, educating families about services, and ensuring seamless transitions of care. You will collaborate closely with physicians, hospitals, and facility staff to identify patients who qualify for hospice and facilitate timely admissions. The ideal candidate is an experienced RN with outstanding assessment, communication, and relationship-building skills, dedicated to providing dignity, comfort, and support at the end of life.
Key Responsibilities
• Act as the primary clinical liaison between hospice care, patients, families, and referral sources.
• Perform thorough clinical assessments to determine hospice eligibility and appropriateness of care.
• Educate patients, families, and healthcare providers on hospice philosophy, services, and options available.
• Coordinate and ensure smooth, timely admissions into hospice care.
• Collaborate with physicians, hospitals, and facilities to identify patients eligible for hospice services.
• Document assessments, referrals, and all communications accurately and promptly in the electronic medical record (EMR).
• Support interdisciplinary team care planning with valuable clinical insights gained from field interactions.
• Ensure compliance with hospice, regulatory, and documentation standards.
