Now Hiring: Flexible PRN RNs
Your Role as a Hospice RN/Care Manager:
• Serve as the primary point of contact for patients and families, offering guidance throughout their hospice journey.
• Deliver skilled nursing care and document progress based on individualized Plans of Care in patients' homes or facilities.
• Conduct initial and ongoing assessments to monitor patient conditions and adjust care plans accordingly.
• Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, and chaplains—to refine personalized care strategies.
• Provide emotional, physical, and spiritual support to patients and their loved ones, including education and comfort during bereavement.
• Teach caregivers safe care techniques for their loved ones while offering emotional reassurance.
• Communicate significant updates on patient conditions to ensure timely interventions.
• Maintain accurate clinical documentation and adhere to infection control standards.
• Support the organization’s quality improvement efforts and scheduling initiatives to provide consistent and compassionate care.
• Assist patients and families in understanding topics such as medication management, hospice services, symptom control, and end-of-life expectations.
Key Responsibilities:
• Conduct comprehensive patient assessments, and provide education to patients and their families.
• Implement skilled nursing interventions and manage symptoms according to care goals.
• Create and update individualized care plans based on patient needs.
• Coordinate care with interdisciplinary teams and consult with physicians as necessary.
• Educate patients and families on disease processes, self-care, and nutritional needs.
• Notify relevant providers of any changes in patient conditions.
• Facilitate smooth transitions in care by recognizing palliative and hospice referrals as necessary.
• Document clinical records promptly, ensuring accuracy and completeness.
• Foster teamwork and collaboration within the interdisciplinary group for patient care.
• Participate in on-call and patient support activities as needed.
Qualifications:
• Education: Registered Nurse
• Experience: Minimum of one year as an RN in acute care, oncology, long-term care, hospice or home health preferred; clinical internships may be considered.
• Licenses and Certifications:
• Active nursing license in the applicable state
• CPR Certification
• Valid driver's license and current automobile insurance
