Physical Therapist Home Health - Ankeny, Iowa

A skilled and dedicated Home Health Physical Therapist is needed to deliver compassionate, patient-centered care in the home setting. This full-time, 13-week contract role focuses on supporting adults recovering from illness, surgery, or managing chronic conditions, with an emphasis on maximizing functional independence and safety.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM

• Field-based position providing physical therapy in patients homes

• Caseload expectation: approximately 2532 points per week (typically 57 visits per day)

• Daily travel within an assigned territory (mileage reimbursement provided)

• Electronic documentation using Home Care Home Base (HCHB)

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including completion of OASIS assessments

• Develop and implement personalized treatment plans to address mobility, strength, pain management, balance, and safety

• Provide interventions such as therapeutic exercise, gait training, transfer training, and fall prevention

• Assess patient progress, update plans, and communicate changes to the care team and physicians

• Instruct patients and caregivers on exercises, assistive devices, and safety in the home

• Collaborate closely with nurses, occupational and speech therapists, and other team members

• Comply with regulatory, policy, and Medicare home health standards

Preferred Qualifications:

• Active Physical Therapist (PT) license in good standing for the state

• At least 1 year of recent Home Health PT (field-based) experience

• Experience with Home Care Home Base (HCHB) and OASIS required

• Strong clinical decision-making, organizational, and communication abilities

• Reliable transportation, valid drivers license, and auto insurance

• Ability to work productively and independently across varied home environments
