Our innovative hospice and palliative care programs play a vital role in easing the burdens associated with managing serious illnesses.

Your Role:
• Deliver high-quality, compassionate NP services to pediatric patients enrolled in hospice. You will also support pediatric palliative care consultations as part of MJHS Hospice and Palliative Care.
• Engage with patients in community settings, and occasionally provide consultation in long-term care environments or hospitals.
• Conduct in-person and telehealth visits, ensuring patient-centered care.
• Implement comfort care aligned with best practices, which includes patient history-taking, thorough examinations, care planning, treatment recommendations, communication, and documentation.
• As a hospice NP, you will coordinate closely with the team physician and manager, conducting face-to-face evaluations for hospice eligibility and managing the care of patients in need of urgent medical attention.
• Contribute to training for the hospice team, offering support for interdisciplinary collaboration.
• Play a key role in developing a teaching curriculum focused on pediatric aspects of hospice care to enhance the skills of our hospice workers.
• Utilize an evidence-based approach in both direct care and support to other clinicians while adhering to relevant materials and policies.
• Participate in quality improvement initiatives as needed.

Qualifications:
• Master's degree in nursing from an accredited Nurse Practitioner program.
• At least 1 year of clinical experience as a Nurse Practitioner preferred.
• Experience in a specialist-level palliative care team or hospice setting is strongly preferred.
• Prior experience in pediatric nursing or as a pediatric Nurse Practitioner is highly desirable.
• Current NYS NP License (Certified Pediatric, Family, or Palliative Care Nurse Practitioner).
• CPR-BLS certification.
• Valid driver's license and access to a car.
• Hospice & Palliative Care certification is encouraged by your second year of employment.
• Proficient in Microsoft Word and Excel.
• Flexibility to work hours that meet the needs of the position.
• Adept at coordinating resources and personnel in a comprehensive, interdisciplinary team environment.
• Willingness to travel within the agency's geographic territory as required.
• Strong communication, teaching, and listening skills.
• Cultural competency to work non-judgmentally with diverse patients and families.
• A commitment to accountability and excellence.
• Excellent organizational, critical thinking, and problem-solving skills.
