Nurse Practitioner for Hospice and Palliative Care (Per Diem)

Job Description

Join our compassionate team and make a meaningful impact in the lives of patients with life-limiting conditions. We offer competitive per diem rates as follows:

In-person initial visit: $180
In-person follow-up visit: $100
Telehealth follow-up visit: $70

Your role will involve providing expert palliative care to patients either in community settings or facilities, working collaboratively with a dedicated group of MDs and NPs. As a vital member of our team, you will be involved in:
• Implementing specialist palliative care consultation services.
• Maintaining effective communication with referral sources.
• Performing timely and accurate documentation.
• Identifying self-learning needs and pursuing continuing education.
• Demonstrating professionalism and respect when interacting with patients, families, and fellow healthcare professionals.
• Engaging in clinical and administrative tasks within your scope of practice as needed.

We are looking for candidates with a Master's degree in Nursing and completion of an accredited Nurse Practitioner Program. A minimum of one year of clinical experience as a NP is preferred, ideally in hospice, palliative care, or geriatric nursing.

Requirements include:
• Valid New York State RN licensure and certification as an Adult, Family, Gerontology, or Palliative Care Nurse Practitioner.
• CPR-BLS certification and a valid NYS Driver's license are preferred.
• Hospice & Palliative Care certification is strongly encouraged by the second year of employment.
• Proficiency in Microsoft Word and Excel.
• A flexible schedule to meet patient care needs.
• The ability to coordinate interdisciplinary team resources and personnel.
• Effective communication, teaching, and listening skills.
• A non-judgmental approach to working with diverse patient populations.
• A high level of accountability and strong organizational skills.
• Critical thinking and independent problem-solving abilities.
