Job Description
Join our innovative hospice and palliative care team, where we make a significant impact on patients dealing with life-limiting conditions. We offer a comprehensive range of services, both in the community and facility-based, to support patients and their families as they navigate the challenges of serious illnesses and end-of-life care.
In this role, you will provide compassionate care by visiting hospice patients in their homes or nursing facilities. You will document each visit in the Electronic Medical Record and communicate any changes in the patient's condition to the Registered Nurse.
This position includes a commitment to be On Call 2-3 times per month and on 2-3 holidays per year.
• Graduate from an accredited Licensed Practical Nurse Program
• A minimum of one year of experience as an LPN in Med/Surg nursing and one year of experience in Hospice or Oncology nursing is required
• Proficient PC skills are essential
• A valid driver's license and a vehicle are required for all patient visits, except in Manhattan
