Key Responsibilities
• Provide on-call triage for home health and hospice patients through both phone consultations and in-person visits.
• Conduct timely patient assessments and ongoing monitoring in the home setting.
• Manage patient symptoms and administer medications and treatments as prescribed.
• Work collaboratively with physicians and the interdisciplinary team to tailor and adjust care plans.
• Maintain accurate and prompt documentation of all assessments, interventions, and communications.
• Educate patients and their families on disease processes, medication regimens, and home care best practices.
• Assist in safe discharge planning, promoting patient comfort and safety at home.
• Respond effectively to urgent after-hours needs, including evenings, nights, weekends, and holidays.
