On-Call Home Health & Hospice Registered Nurse

Addus Family Company is seeking an On-Call Registered Nurse to support home-based patients during evenings, nights, weekends, and holidays. The RN will triage after-hours calls, provide clinical guidance, conduct urgent home visits, perform assessments, manage medications, and coordinate with hospice, home health, and personal care teams. Responsibilities include updating care plans, documenting in the EMR, collaborating with caregivers and families, and ensuring compliance with quality and safety standards. This mission-driven role supports patients in remaining safely and comfortably at home.

Responsibilities
• Triage after-hours patient and family calls and provide clinical guidance
• Perform urgent in-home nursing visits as needed
• Conduct comprehensive patient assessments and monitor conditions
• Administer and reconcile medications and treatments per care plan
• Update and maintain accurate documentation in EMR systems
• Collaborate with hospice, home health, and personal care teams
• Communicate with physicians, caregivers, and families on plan of care
• Ensure compliance with clinical, safety, and regulatory standards
• Participate in on-call rotation, including evenings, nights, weekends, and holidays
• Educate patients and caregivers on disease management and symptom control
