Key Responsibilities
• Deliver direct nursing care for hospice patients in their homes under the supervision of RNs and physicians.
• Administer medications and treatments focused on pain relief and symptom management.
• Monitor and document patient conditions, promptly reporting any changes to the interdisciplinary care team.
• Conduct wound care, catheter care, and additional skilled nursing procedures as prescribed.
• Educate patients and their families on disease processes, comfort measures, and medication regimens.
• Work collaboratively with RNs, physicians, social workers, chaplains, and aides to implement comprehensive care plans.
• Address the emotional and psychosocial needs of patients and families during end-of-life care.
• Maintain accurate and timely electronic documentation, adhering to agency and regulatory standards.
• Participate in an on-call rotation for urgent visit coverage when necessary.
• Follow established infection control, safety, and ethical guidelines in all nursing activities.
Essential Skills
• Experience in hospice and palliative nursing care.
• Proficiency in medication administration and effective pain management strategies.
• Skilled in wound care and maintaining skin integrity.
• Ability to conduct symptom assessments and monitor vital signs.
• Familiarity with electronic health record (EHR) documentation.
• Strong coordination and teamwork capabilities with interdisciplinary teams.
• Commitment to patient and family education.
• Knowledge of infection control principles and safety procedures.
• Understanding of home health nursing practices.
• Effective time management and prioritization skills.
