Key Responsibilities as a Hospice RN/RN Case Manager:
• Act as the primary contact for patients and families, guiding them through their hospice journey with care and understanding.
• Provide skilled nursing care based on each patient's unique Plan of Care in their home or wherever they reside.
• Conduct initial and ongoing assessments to monitor patient condition and adjust care accordingly.
• Collaborate with an interdisciplinary team, including physicians, LPNs, CNAs, social workers, and chaplains, to enhance personalized care plans.
• Offer emotional, physical, and spiritual support to patients and their loved ones, providing education and bereavement guidance.
• Instruct caregivers in safe and confident care practices, ensuring they feel supported and informed.
• Communicate patient updates to the care team to facilitate timely interventions.
• Maintain accurate clinical documentation and adhere to infection control and compliance protocols.
• Contribute to quality improvement initiatives and assist with scheduling to enhance the consistency and efficiency of care.
• Educate patients and families about critical topics including medication administration, hospice philosophy, symptom management, and end-of-life expectations.

Essential Functions:
• Conduct comprehensive patient assessments and provide appropriate education to patients and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with patient care goals.
• Develop and update individualized care plans, including patient visit frequency that addresses individual needs.
• Coordinate care within interdisciplinary teams and consult with physicians and nurse practitioners as necessary.
• Educate patients and caregivers on self-care techniques and nutritional requirements related to their conditions.
• Report any changes in patient condition to the relevant healthcare provider promptly.
• Facilitate continuity of care through appropriate referrals to home health, palliative, and hospice services.
• Document clinical records accurately, including assessments, physician orders, progress notes, and care plans.
• Build effective relationships with the interdisciplinary group and involve team members when necessary.
• Participate in on-call services as needed, delivering services with clinical competence and responsiveness.

About You:

To succeed in this role, you must effectively perform each essential duty. The following are requirements for the position:
• Education: Registered Nurse
• Experience: A minimum of one (1) year in an RN role within acute care, oncology, long-term care, hospice, or home health. School internships and clinical hours may count as experience in specific cases. Hospice or palliative care experience is preferred.
• Licenses and Certifications: Current nursing license in the state where you will work, CPR Certification, valid driver's license, and current auto insurance.
• Specialized Skills: Strong knowledge of caring for terminally ill patients and their families, excellent patient assessment skills, understanding of community resources, and proficiency in documentation and program management.

Working Conditions:
• Work in patients' homes or institutional settings, with potential exposure to infections, communicable diseases, and hazardous materials.
