Shift Schedule: One week on, one week off (Friday evening to Thursday morning).

We are looking for a passionate Afterhours RN Care Manager to deliver high-quality hospice care outside of standard business hours. In this vital role, you will ensure that patients and their families receive continuous, compassionate care during nights, weekends, and on-call hours.

Role Overview
As an Afterhours Care Manager RN, you will assess and coordinate the needs of patients and families during after-hours shifts while maintaining primary clinical oversight for your assigned patients. Your responsibilities will include routine visits, admissions, urgent calls, death visits, and any additional tasks outlined in the patient's care plan. As an integral member of the Interdisciplinary Group (IDG), you will collaborate with physicians, families, and hospice teams to promote cohesive care delivery. Additionally, you will be involved in administrative duties, audit workflows, and ensure compliance with regulatory standards.

Key Responsibilities
• Deliver appropriate hospice services to eligible patients and families.
• Provide direct in-person patient care during weekends, nights, and on-call shifts, encompassing assessments, admissions, and visits.
• Conduct comprehensive assessments of physical, emotional, and spiritual needs.
• Collaborate on the Hospice Plan of Care with physicians and IDG members.
• Educate patients and families on disease progression, symptom management, and the hospice philosophy.
• Communicate significant changes in patient condition promptly to relevant medical staff.
• Maintain accurate and timely clinical documentation, including assessments and care plans.
• Participate in scheduling modifications and support administrative compliance activities.
• Support infection control standards and ensure adherence to universal precautions.
• Provide leadership and mentorship to field staff while conducting supervisory visits when necessary.
• Engage in required meetings and training sessions.
• Promote the organization’s core values and foster a culture of compassion and professionalism.
• Others duties as assigned.

Compensation
Salary range: $71,450 - $89,335, with compensation determined by factors such as location, experience, education, and skills.

Qualifications
• Current RN license in the state of employment.
• Graduation from an accredited nursing program.
• A minimum of one year of RN experience in hospice, acute care, oncology, or home health.
• Compliance with any state-specific requirements.

Licenses and Certifications
• Active RN license
• CPR certification (in-person)
• Valid driver's license with reliable transportation
