This role offers flexible scheduling options as a Case Manager:
• Monday-Thursday, 10am-9pm, on-call from 9pm-8am
• Wednesday-Saturday, 10am-9pm, backup on-call from 9pm-8am
• Monday-Friday, 8am-5pm with limited backup on-call
Note: Additional incentives are offered for on-call duties.
What You'll Do as a Hospice RN / RN Case Manager:
• Serve as the primary point of contact for patients and families, guiding them through their hospice journey with compassion.
• Deliver skilled nursing care in accordance with each patient's individualized Plan of Care within their home or assisted living environment.
• Conduct thorough initial and ongoing assessments to monitor patient conditions and adjust care plans as required.
• Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, chaplains, and more—to enhance and personalize care plans.
• Provide emotional, physical, and spiritual support to both patients and their loved ones, offering education, comfort, and bereavement guidance.
• Empower caregivers with practical training to ensure confident and safe care for their loved ones.
• Update the care team on any significant changes in patient condition to ensure prompt interventions.
• Maintain accurate and current clinical documentation while adhering to infection control and compliance standards.
• Support quality improvement initiatives to ensure our care remains consistent, compassionate, and effective.
• Guide patients and families through essential topics such as:
• Medication administration
• Hospice services and philosophy
• Symptom and pain management
• End-of-life processes and expectations
Essential Functions:
• Perform comprehensive patient assessments and reassessments, including in-person and telephonic screenings, while educating patients and caregivers.
• Administer skilled nursing interventions and manage symptoms according to patients' goals of care.
• Craft and update individualized care plans, addressing patient needs and identifying potential risks.
• Facilitate care coordination with interdisciplinary teams, engaging physicians and nurse practitioners as needed.
• Provide education for patients and caregivers on disease processes, self-care, end-of-life care, medications, and nutritional needs.
• Communicate any changes in patient status to the designated healthcare provider promptly.
• Recognize and support the continuity of care across home health, palliative, and hospice referrals.
• Document all clinical interactions within established timelines, including assessments, physician orders, progress notes, and care plans.
• Build effective working relationships with the interdisciplinary group, involving the team when necessary to achieve optimal outcomes.
• Participate in on-call services, responding with clinical competence and sensitivity.

About You:
To excel in this role, you must meet the following qualifications:
• Education: Must be a Registered Nurse.
• Experience: At least one year of RN experience in acute care, oncology, long-term care, hospice, or home health. Clinical internships may be considered in individual cases.
• Prior hospice or palliative care experience is preferred.
Licenses, Certifications, and Registration:
• Active nursing license in the state of employment
• CPR Certification
• Valid driver's license and automobile insurance
Specialized Knowledge and Skills:
• Understanding of the needs of terminally ill patients and their families; ability to apply knowledge effectively in practice.
• Excellent patient assessment and communication skills.
• Familiarity with community resources, documentation standards, and regulatory guidelines (Medicare, Medicaid, ACHC).
• General nursing practice knowledge and program management skills.
Working Conditions:
• Work will mostly occur in patients' homes and institutional settings.
• May involve exposure to infectious diseases, hazardous materials, and other risks.
Personal Traits and Qualities:
• Empathy toward individuals facing life and death issues.
• Ability to remain calm and efficient during crisis situations.
• Strong communication skills with diverse populations.
• Professional maturity, cooperation, flexibility, and tact in personal interactions.
Equipment/Tools/Work-Aids:
• Ability to operate nursing assessment tools and basic office equipment. Reliable transportation is required.
