Breast Health Navigator - Full-Time

This role is for a registered nurse (RN) specializing in patient navigation to enhance patient experience and clinical outcomes.

Responsibilities include collaborating with physicians and interdisciplinary teams to triage, coordinate, and manage patient care, while educating patients and serving as a communication point for families.

Key duties involve developing and implementing navigation activities, promoting shared decision-making, and addressing health literacy and communication needs to support patient engagement.

Qualifications include graduation from an accredited nursing program, California RN licensure, and BLS certification.

Preferred Experience Includes Five Years Of RN Navigator Work.

Skills required are excellent communication, collaboration, and patient-centered care.
