Position Overview: As a Weekend PRN Registered Nurse, you will play a crucial role in delivering compassionate hospice care during after-hours, including nights and weekends. Your primary responsibilities will include:
• Triage patient needs over the phone until an in-person visit can be arranged.
• Respond promptly and effectively to urgent crisis calls from patients and their families.
• Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral, when assigned.
• Perform after-hours visits, such as admission visits, death visits, recertification visits, and routine assessments.
• Identify and reassess the physical, psychosocial, emotional, and environmental needs of patients and families, at least every 15 days.
• Document care plans, problem-solving strategies, and the responses of patients and families to hospice services.

Required Qualifications:
• Current unrestricted RN licensure in your state of practice.
• Valid driver’s license, vehicle insurance, and access to reliable transportation or public transport.
Preferred Qualifications:
• Current CPR certification or willingness to obtain within 90 days of hire.
• At least 1 year of clinical experience preferred.
Your pay will be competitive, ranging from $37.40 to $56.10 per hour based on qualifications, education, experience, and local labor market conditions.
