Travel Skilled Nursing Facility Physical Therapist

TLC Allied is seeking a travel Physical Therapist for a Skilled Nursing Facility assignment in Castro Valley, CA. Pay ranges from $2,086 to $2,149 per week, with day shifts scheduled as 5 x 8 hours. The Physical Therapist will assess residents’ mobility, strength, balance, and activities of daily living; develop individualized rehabilitation plans; provide therapeutic exercise, manual therapy, and gait training; monitor progress; document outcomes in the EHR; and coordinate care and discharge planning with nursing, therapy staff, residents, caregivers, and families. Qualifications include a Doctor of Physical Therapy or equivalent degree from an accredited program, applicable state licensure or eligibility, CPR/BLS certification, and experience providing physical therapy in a skilled nursing facility or similar rehabilitation setting.
