TLC Allied is seeking a travel Home Health Physical Therapist in Camden, South Carolina. This day-shift assignment works 5 x 8-hour shifts and pays $2,435–$2,509 per week. The PT provides rehabilitation services in patients’ homes, including assessments, individualized treatment plans, therapeutic interventions, progressive exercise programs, and patient and caregiver education. Responsibilities also include coordinating with physicians, nurses, caregivers, and home health aides and documenting progress and outcomes in the EHR. A DPT or equivalent, current U.S. PT license, and CPR/BLS certification are required. The role calls for 1–3 years of PT experience, with home health experience preferred.
