Therapist / Home Care Clinical Supervisor (OT, SLP Or SW)

Excellent Home Care Services is hiring a Therapist / Home Care Clinical Supervisor (OT, SLP or SW) for our Brooklyn location to manage a team of Field Therapists. The ideal candidate will possess extensive clinical expertise in therapy, with a focus on patient rehabilitation, care plan development, and interdisciplinary collaboration.

Duties:


- Managing a team of Home Therapists
- Serving as the primary point of contact between the Agency and your Therapy Team
- Ensuring Patient satisfaction and delivering services according to Agency policies
- Overseeing the development and implementation of individualized treatment plans for patients.
- Coordinate discharge planning and collaborate with healthcare professionals to ensure seamless patient transitions between care settings.
- Serving as the primary point of contact between the Agency and your Therapy Team
- Familiarity with clinical documentation standards, EMR systems (Electronic Medical Records).
- Ability to collaborate effectively with multidisciplinary healthcare teams.


Requirements:


- Valid and current NYS Therapy License in your specialty
- 2 year of clinical experience
- Certified Home Health Agency (CHHA) experience preferred
- Strong attention to detail
- Excellent oral and written communication skills
- Ability to multitask, meet deadlines, and problem-solve
- Working knowledge of Medicare/Medicaid and other Insurance programs preferred
- Knowledge of Home Care-based therapy services is required for this supervisory role.
