Job Summary
As a Registered Nurse, you will provide efficient and compassionate care to assigned patients, taking the lead in the care team while adhering to established standards, procedures, and individualized patient plans. You will evaluate outcomes based on clinical, financial, and utilization data while maintaining knowledge of hospice palliative care and the various needs associated with end-of-life care.
Your responsibilities include:
• Developing and implementing plans of care in collaboration with the interdisciplinary team
• Conducting thorough assessments of patients in line with regulations
• Establishing service plans and continually evaluating patient needs
• Communicating effectively with patients, families, and the healthcare team
To succeed in this role, you must exhibit critical thinking, strong communication skills, and the ability to motivate both patients and staff. You will be expected to document care clearly and manage competing priorities effectively.
Requirements include:
• Bachelor's degree in Nursing is strongly preferred
• Graduate from an accredited nursing program
• A minimum of two years of clinical experience is preferred
• An unencumbered RN license with the appropriate state nursing board
• A valid driver's license
