Salary Range: 36.50 - 51.00

Our hospice care team is dedicated to meeting the physical, spiritual, and emotional needs of those we serve. We ensure that patients spend their last days in dignity, whether at home or in a warm, home-like inpatient setting.

Job Summary
As a Hospice RN Case Manager, you will provide compassionate care to patients while leading our dedicated staff. Your role will involve:
• Implementing individualized patient care plans
• Evaluating outcomes and integrating clinical, financial, and utilization data
• Understanding hospice palliative care and demonstrating cultural sensitivity towards end-of-life issues
• Communicating effectively with patients, families, and healthcare teams
• Assessing residents and ensuring compliance with regulations
• Creating service plans and evaluating them continuously
• Documenting care clearly and providing education tailored to age-specific needs
You must exhibit independence, critical thinking, and excellent organizational skills. A self-motivated approach is essential, as well as the ability to prioritize tasks effectively.
Work primarily during day hours, with some weekends and occasional evenings. Be prepared for potential exposure to communicable diseases and other environmental stressors.
Qualifications
- Bachelor's degree in Nursing strongly preferred.
- Graduate from an accredited nursing program (CCNE, ACEN, or NLN CNEA).
- Minimum of two years' clinical experience preferred.
- Current, unencumbered RN license from the State Board of Nursing, or multistate licensure if in a Nurse Licensure Compact state.
- Possession of a valid driver's license.
