As a Registered Nurse Case Manager, you will take full nursing responsibility for implementing and overseeing the Plan of Care for each hospice patient. You will continuously evaluate and enhance both personal and professional performance to increase productivity and the quality of care provided. Collaborative development of the patient's care plan with other professionals is essential, as well as implementing treatment strategies grounded in scientific nursing theory to support physical, psychosocial, emotional, and spiritual well-being. You will engage with patients and families who are facing end-of-life issues with compassion and understanding.

Primary Responsibilities:
• Conduct initial nursing evaluations to determine hospice eligibility within forty-eight (48) hours of referral.
• Assess and address the physical, psychosocial, and environmental needs of patients and families, re-evaluating no less than every fifteen (15) days.
• Initiate and coordinate comprehensive plans of care.
• Document relevant problems, goals, interventions, and patient/family responses to care.
• Engage with patients, families, attending physicians, and interdisciplinary team members daily to provide integrated care.
• Instruct and supervise patients and families on self-care techniques, ensuring they are empowered in their care process.
• Maintain accurate and insightful clinical notes regarding patients' conditions.
• Demonstrate the ability to provide care in diverse home environments, regardless of socio-economic status or background.

Required Qualifications:
• Current and unrestricted RN licensure in the state of practice.
• Valid driver's license, reliable vehicle insurance, and access to dependable transportation.
• A minimum of 2+ years of full-time RN experience. Note that two years of full-time experience as a licensed practical nurse in hospice care can substitute for this requirement.

Preferred Qualifications:
• 1+ years of clinical experience in a healthcare setting.
• Current CPR certification or the ability to obtain it within 90 days of hire.
