PRN Registered Nurse for Hospice Care

This is a PRN position involving home visits for hospice care.

As the Registered Nurse Case Manager, you will take full nursing responsibility for delivering each patient's Plan of Care. You'll assess and enhance your performance continuously to improve productivity and the quality of care. Collaborating with a team of professionals, you will develop personalized care plans and implement evidence-based treatment strategies that promote the physical, psychosocial, emotional, and spiritual well-being of patients facing end-of-life challenges.

Key Responsibilities:
• Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
• Assess and identify the physical, psychosocial, and environmental needs of patients and families, with regular reassessments at least every 15 days.
• Initiate and coordinate comprehensive care plans.
• Document patient needs, goals, interventions, and responses to hospice care meticulously.
• Collaborate daily with patients, families, attending physicians, and interdisciplinary team (IDG) members to provide holistic care.
• Guide and supervise patients and families in self-care techniques when appropriate.
• Maintain accurate and relevant clinical notes documenting the patient's condition.
• Adapt to diverse home situations regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Required Qualifications:
• Current and unrestricted RN licensure in the state of practice.
• Valid driver’s license, vehicle insurance, and reliable transportation, or access to public transportation.

Preferred Qualifications:
• 1+ years of clinical experience.
• Current CPR certification or willingness to obtain within 90 days of hire.

Pay is determined based on various factors including local labor markets, education, experience, and certifications. In addition to salary, we offer a comprehensive benefits package, including incentive programs, stock purchase options, and 401k contributions (subject to eligibility). Our salary range for this role is between $87,560 and $131,340 annually based on full-time employment. We comply with all applicable minimum wage laws.
