As a Hospice Nurse Practitioner, you will play a crucial role in delivering expert palliative care, symptom management, and supportive services tailored to our hospice patients. Your contributions will be vital in enriching the lives of patients and their families during challenging times.

Key Responsibilities:
• Conduct face-to-face visits with qualifying hospice patients within designated timeframes, documenting findings and reporting to the Hospice Medical Director.
• Coordinate care in collaboration with the Hospice Medical Director and primary physician, recommending services to ensure comprehensive patient care.
• Develop and implement individualized plans of care, maintaining clear documentation of patient conditions, goals, and responses to treatment.
• Educate and guide patients and families on self-care practices to enhance their comfort and independence.
• Maintain accurate clinical records and report any patient concerns to the Administrator and Hospice Medical Director.

Required Qualifications:
• Active, unrestricted nursing license in your state of residence.
• Certification as a clinical Nurse Specialist or Nurse Practitioner in the relevant state.
• Certification from a recognized national body such as ANCC, NBCHPN, or AANP.
• Valid driver's license and vehicle insurance, and access to reliable transportation.
• Current CPR certification.
• A minimum of 5 years nursing experience.

Preferred Qualifications:
• Experience in oncology, hospice, geriatrics, or intensive care nursing.
• Palliative Care ARNP certification.
• Proven ability to work effectively within an interdisciplinary team.

Your compensation will be competitive, influenced by various factors, including local labor market conditions, your education, experience, and certifications.
