Coverage Territory: Bloomington, Pontiac, Lincoln, and surrounding areas
Schedule: PRN as needed
Salary Range: $36.00 - $43.00 an hour

Your Role and Responsibilities
• Deliver comprehensive nursing care throughout the full nursing process within your scope of practice.
• Conduct thorough assessments of patients and families, contributing to personalized care plans.
• Provide all visit types as needed to address the needs of patients and families effectively.
• Administer medications and treatments following physician orders.
• Identify nursing diagnoses from health assessment data and respond appropriately to patient needs.
• Document accurate and timely clinical notes, including pain and symptom management outcomes.
• Communicate any changes in patient condition to physicians and obtain necessary orders.
• Coordinate with community health partners to ensure seamless care transitions for patients.
• Arrange for the necessary equipment and services to be available for patient care.
• Continuously enhance your clinical skills through ongoing education and training sessions.

What You Bring
• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Preferred clinical experience in hospice, home health, or a related setting.
• Flexibility and confidence in managing diverse patient assignments.
• Strong assessment skills and sound clinical judgment under pressure.
• Excellent documentation practices and communication abilities.
• Valid driver's license and willingness to travel locally.
