Job Description
Coverage Territory: Bloomington, Pontiac, and surrounding areas
Schedule: 8 AM-8 PM, alternating 4-day and 3-day weeks including weekends in rotation
Salary Range: $75,000 - $85,000 a year

Your Role
As an RN Case Manager, you will serve as a crucial support for your patients and their families, overseeing care, guiding the interdisciplinary team, and showing up in meaningful ways. Your daily responsibilities will include:
• Conducting thorough and ongoing assessments to address each patient's changing needs
• Creating and refining personalized care plans in partnership with physicians and care teams
• Administering medications and treatments as directed by physician orders
• Maintaining accurate, timely, and meaningful clinical documentation
• Acting as a communication link between families, physicians, and community health partners
• Educating patients and caregivers to empower their confidence and support
• Participating in on-call rotation as per team policy

Qualifications You Bring
• Active RN license in the state of employment
• Graduate of an accredited Nursing program
• Preferred: 1+ years of experience in Hospice, home health, or related fields
• Empathy for patients and families navigating end-of-life care
• Strong clinical judgment with the ability to work independently in the field
• Outstanding communication skills for effective collaboration
