We are seeking a Per Diem Registered Nurse Case Manager who will take full responsibility for delivering personalized nursing care to our hospice patients. You will play a vital role in ensuring the quality of care and continuously improve personal and professional performance. By adhering to our hospice policies and procedures, you will help ensure we meet the goals and objectives for quality patient care.
Your Key Responsibilities:
• Conduct initial nursing evaluations to determine hospice eligibility within 48 hours of referral.
• Assess and identify the physical, psychosocial, and environmental needs of the patient and their family, re-evaluating these needs at least every 15 days.
• Initiate and coordinate the plan of care while documenting patient conditions with accurate and relevant clinical notes.
• Record problems, establish appropriate goals, interventions, and monitor patient/family responses to care.
• Collaborate daily with the patient/family, attending physician, and other IDG members for comprehensive care.

Required Qualifications:
• Current unrestricted RN license in your state of practice.
• At least 1 year of clinical nursing experience.
• Valid driver's license, vehicle insurance, and access to reliable transportation or public transport.
Preferred Qualifications:
• Current CPR Certification or ability to obtain it within 90 days of hire.
