Per Diem Nurse Practitioner - Hospice

As a Hospice Nurse Practitioner, you will play a crucial role in providing consultation for palliative care, symptom management, and supportive services tailored to the needs of hospice patients as referred by their physicians.

Key Responsibilities:
• Conduct face-to-face visits with qualifying hospice patients within specified time frames and communicate clinical findings to the Hospice Medical Director.
• Consult and collaborate with the Hospice Medical Director and primary physicians as needed, coordinating comprehensive care for patients.
• Initiate and oversee individualized care plans, documenting patient needs, goals, interventions, and responses to hospice care while working closely with patients, families, and healthcare teams.
• Guide and educate patients and families in self-care techniques when appropriate to enhance their quality of life.
• Maintain precise and relevant clinical notes regarding patient conditions and communicate any needs or issues to the Administrator and Hospice Medical Director.

Required Qualifications:
• Active unrestricted license in your state of residence.
• Current certification as a clinical Nurse Specialist or Nurse Practitioner in practice.
• Valid national certification from recognized bodies such as ANCC, NBCHPN, or AANP.
• Minimum of 5 years of nursing experience.
• Current driver's license and vehicle insurance, along with access to reliable transportation.

Preferred Qualifications:
• Current CPR certification or ability to acquire it within 90 days of hire.
• Experience working collaboratively with an interdisciplinary team.
• Clinical nursing experience in specialties such as oncology, hospice, geriatrics, or intensive care.
• Palliative Care ARNP certification.

Compensation will vary based on factors such as local labor markets, education, experience, and certifications. In addition to competitive pay, we also offer a comprehensive benefits package, incentive programs, stock purchase options, and a 401k contribution (subject to eligibility requirements). The hourly wage for this position ranges from $43.58 to $65.37 based on a full-time schedule.
