As a Per Diem Hospice Admissions Nurse, you will play a crucial role in facilitating the hospice admission process in accordance with federal and state regulations. Your advocacy for patients and their families will ensure their needs are met through hospice services and appropriate referrals to additional resources.

Primary Responsibilities:
• Assess potential patients for hospice eligibility and engage families to understand their needs
• Maintain confidentiality and safety of patient and client clinical records
• Complete admissions and patient care orders efficiently and accurately
• Communicate effectively with physicians, referral sources, patients, families, and the hospice interdisciplinary team regarding new patient admissions
• Coordinate with operational, clinical, and sales leadership to update them on the status of pending referrals

Required Qualifications:
• Current unrestricted RN license in the state of operation
• Minimum of 2 years of experience in hospice, home health, or a related healthcare setting
• Current CPR certification
• Valid Driver's License with vehicle insurance, and access to reliable transportation

Preferred Qualifications:
• Demonstrated fiscal responsibility and accountability
• Proficient computer skills, particularly with Microsoft Word and Excel

Compensation is competitive and based on various factors, including local labor markets, experience, education, and certifications. In addition to a competitive salary, we offer a comprehensive benefits package that includes incentives, recognition programs, equity stock options, and 401k contributions (subject to eligibility requirements). The hourly pay for this role ranges from $81,920 to $122,880 for full-time employment.
