Job Description
Role: Registered Nurse Patient Navigator
Responsibilities
• Guide patients and families through their healthcare journey by coordinating care across clinics and hospital services.
• Provide education on diagnoses, treatments, and care plans to promote understanding and adherence.
• Collaborate with physicians, multidisciplinary teams, and community resources to address barriers to care.
• Support seamless transitions between healthcare settings and advocate for patient needs.
Qualifications & Skills
• Compassionate RN with strong clinical assessment abilities.
• Excellent care coordination, communication, and patient education skills.
• Ability to work effectively in a fast-paced, team-based environment.
