Job Description
Role: Licensed Practical Nurse (LPN) responsible for providing patient care under RN supervision, within scope of practice.
Responsibilities
Assessing patient needs, assisting with treatments, preparing patients for exams, and documenting care.
Administering medications, accessing ports, and performing routine nursing procedures.
Coordinating care with the healthcare team, informing providers of patient condition changes, and updating care plans.
Educating patients and caregivers on self-care, utilizing teach-back methods to ensure understanding.
Accurately completing documentation, adhering to regulatory guidelines, and maintaining communication via voicemail and email.
Participating in staff education, onboarding, and continuing education to stay current in clinical skills.
Serving as a preceptor for new staff and supporting team collaboration.
Qualifications & Skills
High School Diploma or GED, with valid LPN license and BLS certification.
Preferred healthcare experience, especially in oncology or outpatient settings.
Strong assessment, clinical, organizational, problem-solving, and communication skills.
Proficiency in computer use, reliable transportation, and scheduling flexibility required.
Additional Information: The role involves local travel, float responsibilities across sites, and active participation in ongoing professional development. The position emphasizes compassionate, patient-centered care and adherence to organizational policies.
