As a Palliative Care Nurse Practitioner, you will deliver compassionate and direct patient and family care through comprehensive palliative care consultations. Your focus will be on patients with serious illnesses who have a prognosis of one year or less but may not yet be ready or eligible for hospice care.

Key Responsibilities:
• Utilize evidence-based palliative medicine practices to provide ethical and effective patient care that considers physical, emotional, spiritual, and psychosocial needs appropriate for the patient demographics served.
• Employ your clinical expertise to develop effective care plans rooted in palliative care principles.
• Conduct thorough assessments by gathering comprehensive information from all relevant sources, including medical records, family histories, and direct evaluations.
• Perform a detailed palliative care assessment, interpret findings, conduct medication reconciliations, formulate diagnoses, initiate care plans with therapeutic interventions, and evaluate their effectiveness for optimal patient outcomes.
• Encouragement to achieve ACHPN certification after two years of employment or upon fulfilling experience requirements.

Required Qualifications:
• Current RN/APRN licensure in your state of practice.
• Nurse Practitioner/CNS designation with prescriptive authority.
• BCLS certification.
• Current CPR certification.
• At least 5 years of clinical nursing experience.

Preferred Qualifications:
• Advanced practice palliative care certification (ACHPN).
• 1+ years of experience as an APRN in Hospice, Palliative Care, or Oncology.

Your compensation will be determined by various factors, including local labor markets, education, and work experience. In addition to your base salary, we offer a comprehensive benefits package, including incentives, recognition programs, stock purchase options, and 401(k) contributions (subject to eligibility requirements). Our salary range for this role is $90,650 to $135,976 annually based on full-time employment.
