Oncology & Triage Health Communications Specialist for Elder Care

Wilham Middon Associates is hiring a Oncology & Triage Health Communications Specialist for Elder Care in Chicago Heights, Cook County. This is a contract position.
What you will do
• Assess patients on admission and set out a care plan with measurable goals.
• Prioritise a caseload when demand exceeds the time available.
• Work with pharmacy to reconcile medication on admission and discharge.
• Work within a multidisciplinary team to coordinate each patient's pathway.
• Contribute to service improvement projects on the ward or department.
• Manage medicines safely, including controlled drug procedures.
• Administer prescribed treatment accurately and record every dose given.
