As an After Hours Hospice RN, you will respond to patient needs during evenings, nights, weekends, and holidays, offering peace of mind and compassionate support when it matters most.

Responsibilities as a Hospice RN - After Hours/On-Call:
• Report to the Administrator, Executive Director, or Patient Care Manager Senior.
• Deliver direct patient care and support outside of regular business hours.
• Act as the primary point of contact for patients, families, physicians, and the hospice team during your shift.
• Respond promptly and compassionately to urgent calls, patient visits, and symptom management needs.
• Assess patient and family needs to inform clinical decisions and implement the care plan.
• Collaborate with an interdisciplinary team, including LPNs/LVNs, CNAs, and other support staff.
• Accurately document clinical visits and communications.
• Engage in quality assessment and performance improvement initiatives.
• Work independently while ensuring seamless continuity of care with the daytime team.

Qualifications - What You Bring:
• Current RN license in the state of employment (or eligibility to obtain).
• At least 1 year of nursing experience.
• A compassionate and reliable disposition, particularly in urgent care scenarios.
• Strong clinical judgment to support patients in emotionally charged environments.
• Comfortable providing care in various settings, including patients' homes and facilities.
• Valid driver's license, automobile insurance, and reliable transportation.
• Current CPR certification.

Preferred Experience (but not mandatory):
• 1+ years of experience in hospice, home health, oncology, or palliative care.
• Familiarity with symptom management, triage, and after-hours home-based care.
• Prior experience in on-call or overnight nursing positions.
• Strong documentation skills and familiarity with electronic medical records (EMR).
