We are looking for a compassionate and dedicated Afterhours RN Care Manager to deliver high-quality hospice care during nights, weekends, and on-call hours. This role is vital in ensuring seamless coverage and continuity of care for our patients and their families when they need it most.

About the Position
As an Afterhours Care Manager RN, you will assess and coordinate the needs of patients and families during after-hours shifts. You will maintain primary clinical oversight for assigned patients, perform routine visits, handle admissions, death visits, manage urgent after-hours calls, and deliver care as per the patient's plan of care. You will play a crucial role as an active member of our Interdisciplinary Group (IDG) to facilitate coordination among physicians, families, and the hospice team while ensuring compliance and quality in care delivery.

Key Responsibilities:
• Deliver compassionate hospice services to eligible patients and families.
• Provide hands-on patient care including assessments, admissions, routine visits, death visits, and live discharges during on-call and after-hour shifts.
• Conduct comprehensive assessments of physical, emotional, psychosocial, and spiritual needs.
• Support the IDG process with chart reviews and effective communication.
• Collaborate with IDG members to create and update the Hospice Plan of Care.
• Educate patients and families on disease progression, symptom management, medications, and the hospice philosophy.
• Promptly communicate any significant changes in patient conditions to the appropriate physicians.
• Maintain accurate and timely clinical documentation of care provided.
• Assist with rescheduling or reassignment of visit requests based on triage needs.
• Participate in scheduling modifications and manage service codes and clinician assignments efficiently.
• Support administrative tasks, including workflow audits and documentation collection for compliance.
• Engage in TPE reviews, QAPI endeavors, and regulatory compliance efforts.
• Uphold infection control standards and universal precautions in all care practices.
• Provide guidance and leadership to field staff and conduct supervisory visits as necessary.
• Engage in required meetings, after-hours updates, and continuous training.
• Show proficiency in pain and symptom management for hospice and end-of-life care.
• Promote core values of compassion, professionalism, and teamwork.
• Perform other duties as assigned.

About You:

Education and Experience:
• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• A minimum of one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Compliance with state-specific requirements.

Licenses and Certifications:
• Active RN license.
• CPR certification (in-person required).
• Valid driver's license and reliable transportation with current auto insurance.

Skills and Competencies:
• In-depth understanding of hospice principles and Medicare/Medicaid regulations.
• Ability to perform thorough patient assessments and create tailored care plans.
• Strong communication skills with patients, families, physicians, and interdisciplinary team members.
• Excellent time-management and prioritization abilities in fast-paced settings.
• Proficient in EMR systems (HomeCare HomeBase preferred), Microsoft Office, and mobile technologies.
• Commitment to confidentiality, integrity, and quality outcomes.
• Effective leadership skills to support field staff.
• Ability to adeptly manage administrative, documentation, and compliance responsibilities.
