On Call Registered Nurse - Hospice Care

Heartland Hospice is searching for a dedicated On Call Registered Nurse to deliver compassionate hospice and palliative care to patients in home and facility settings. As a vital member of our interdisciplinary team, you will play a crucial role in providing comfort and dignity at the end of life. Your responsibilities will include triaging after-hours calls, conducting urgent visits, and managing symptoms effectively.

Key Responsibilities:
• Provide after-hours and on-call hospice nursing care in homes and facilities.
• Triage calls, assess patient status, and determine the urgency of visits.
• Perform urgent visits and complete thorough head-to-toe assessments.
• Administer medications and treatments aimed at ensuring comfort and effective pain control.
• Implement and update individualized hospice care plans in collaboration with the team.
• Educate and support patients and their families regarding end-of-life care and expectations.
• Work closely with physicians, social workers, chaplains, and other team members.
• Document assessments, interventions, and outcomes accurately in the EMR system.
• Coordinate transitions of care and communicate significant changes to case managers.
• Adhere to hospice policies, regulatory standards, and infection control guidelines.
