Heartland Hospice, part of ProMedica Hospice, is seeking a dedicated On Call Hospice Nurse for a rewarding 7on/7off schedule. In this pivotal role, you will deliver compassionate after-hours and weekend hospice nursing care to patients in their homes and various facilities. Your responsibilities will include triaging urgent calls, performing symptom and pain assessments, administering medications, and collaborating with our interdisciplinary team to provide the best possible care. You will play a crucial part in educating and supporting patients and their families, ensuring that care is documented accurately, and working closely with physicians, social workers, and chaplains to uphold comfort, dignity, and high-quality end-of-life care.

Key Responsibilities
• Deliver after-hours and weekend hospice nursing care in both home settings and facilities.
• Triage and promptly respond to urgent calls from patients and their families.
• Conduct thorough assessments of symptoms, pain, and medication needs.
• Administer medications and treatments in accordance with the care plan and physician directives.
• Coordinate care updates with the interdisciplinary hospice team to ensure comprehensive patient support.
• Educate patients and families about disease processes, available medications, and comfort measures.
• Document all assessments, interventions, and outcomes accurately and in a timely manner.
• Address emotional and spiritual needs by collaborating with social workers and chaplains.
• Participate in on-call rotations and maintain a high standard of timely response.
• Ensure that all care aligns with hospice philosophy, safety standards, and regulatory requirements.

Required Skills
