Hospice Registered Nurse

Heartland Hospice, part of ProMedica Hospice, is seeking an On Call Hospice Nurse for a 7on/7off schedule. In this role, you will provide after-hours and weekend hospice nursing care to patients in homes and facilities. Responsibilities include triaging urgent calls, performing symptom and pain assessments, administering medications, and coordinating interventions with the interdisciplinary team. You will educate and support patients and families, document care accurately, and collaborate with physicians, social workers, and chaplains to ensure comfort, dignity, and high-quality end-of-life care.

Responsibilities
• Provide after-hours and weekend hospice nursing care in homes and facilities
• Triage and respond to urgent patient and family calls
• Conduct symptom, pain, and medication assessments
• Administer medications and treatments per care plan and physician orders
• Coordinate care and updates with the interdisciplinary hospice team
• Educate patients and families on disease process, medications, and comfort measures
• Document assessments, interventions, and outcomes accurately and promptly
• Support emotional and spiritual needs by collaborating with social workers and chaplains
• Participate in on-call rotation and maintain timely response standards
• Ensure care aligns with hospice philosophy, safety standards, and regulatory requirements
