Occupational Therapist (OT) - Home Health



Location: Indianapolis, IN and Surrounding Areas



Schedule: Monday-Friday, 8:00 AM-4:30 PM



Weekly Hour Guarantee: 40 Hours



Productivity Expectation: 25-30 Points Per Week



On-Call/Weekend Requirement: None



Mileage Reimbursement: Yes, mileage is reimbursed between patient visits (from first patient to last patient).



Position Overview



We are seeking a dedicated and experienced Home Health Occupational Therapist (OT) to provide high-quality, patient-centered care throughout the Indianapolis metropolitan area and surrounding communities. The ideal candidate will have prior home health experience and be passionate about helping patients regain independence, improve functional abilities, and safely remain in their homes.



As a Home Health OT, you will evaluate patients, develop individualized treatment plans, and collaborate with the interdisciplinary care team to maximize patient outcomes and quality of life.



Key Responsibilities



Conduct comprehensive occupational therapy evaluations in the home setting.



Develop and implement individualized treatment plans based on patient needs and goals.



Complete Start of Care (SOC) visits, reassessments, recertifications, and discharge evaluations as needed.



Assess and improve patients' ability to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs).



Provide therapeutic interventions to improve strength, mobility, coordination, balance, and functional independence.



Recommend adaptive equipment, assistive devices, and home modifications to enhance safety.



Educate patients and caregivers on therapeutic techniques, safety measures, and home exercise programs.



Collaborate with physicians, nurses, physical therapists, speech therapists, and other healthcare professionals to coordinate care.



Maintain accurate, timely, and compliant clinical documentation.



Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.



Required Qualifications



Current and unrestricted Occupational Therapist (OT) license.



Prior Home Health Experience Required.



Strong clinical assessment and treatment planning skills.



Experience with electronic medical record systems; Homecare Homebase (HCHB) experience preferred.



Excellent communication and patient education skills.



Reliable transportation and ability to travel throughout the assigned coverage area.
