HarmonyCares, a leading physician-led provider of innovative home-based primary and palliative care, is looking for a dedicated Advanced Practice Provider to join our team. Key Responsibilities:
• Deliver compassionate home-based and telehealth primary and palliative care to patients with complex medical needs.
• Perform comprehensive assessments, diagnose conditions, and manage ongoing care for both acute and chronic illnesses.
• Collaborate with physicians and an interdisciplinary team to develop, implement, and adjust individualized care plans that address patient needs.
• Facilitate care transitions to reduce unnecessary emergency visits and hospitalizations, ensuring continuity of care.
• Educate patients and families about conditions, medications, and effective self-management strategies for better health outcomes.
• Accurately document all patient encounters, assessments, and care plans in our Electronic Health Record (EHR) system.
• Engage in interdisciplinary case conferences and participate in quality improvement initiatives to enhance care delivery.
• Uphold compliance with clinical, regulatory, and organizational standards to ensure high-quality care.
• Support end-of-life and palliative care goals, aligning with the preferences of patients and their families.
Essential Skills:
• Expertise in primary care clinical assessment and diagnosis.
• Experience in managing chronic diseases and providing palliative care.
• Strong skills in care planning and coordination among multidisciplinary teams.
• Proficient in utilizing telehealth technologies and remote patient monitoring.
